Please give as much information as possible. If there is inclement weather that cancels league we need a way to get in touch with the captain and/or players. Email is the best way with phone numbers as a 2nd. Please provide phone and email for as many players as possible.

Notes:

Winner of the lag goes on top
A "W" or "L" goes in the game column
The number of games WON goes in the W-L column
A single mark in the bonus columns for each accomplishment
Forfeits are ALWAYS the last game. The team receiving the forfeit MUST put a players name against the forfeit but that player must be present at the time and ready to play if needed.
Compare to the other team before you turn it in
260 points total for the team is maximum for a four person team

This score sheet has room for five players. Columbia and Jeff City have four man teams so you will only use a four player score sheet. This is for example only.

You can use X's, W's for the winner and blank or L's for the loser. You can use X's for the loser ONLY if you do not use X's for the winner.
